You should update this every year, or when your information changes.

Arrive 15 minutes before your appointment time.

Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.

Let us know as soon as possible if you have to cancel.

Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.

Payment will be due at the time of your appointment.

Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 25% discount.

I'm a new patient.

For less time in the waiting room, fill out your new patient forms before your appointment.

Get the forms.

I need to refill a prescription.

Bring all current medications in the prescription bottles to each office visit. 

Call your office location for any refill requests. We may ask you to come in for a visit before your refill if your cardiologist hasn’t examined you recently. 

I'm waiting on test results.

You'll receive your test results in 7 to 10 days by mail or telephone.

I'm waiting on a referral.

Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.

I need to fax information.

Please send your fax to the appropriate number.
